Path Assist's Community Care Assessments are customized to address each customer's unique needs. Guided by public health practitioners, our process includes community representation at each stage of our research process. Our process uplifts the voices of community members with lived experience of health and social barriers and those dedicated to serving them in community-based service organizations.

What's Involved

This short-term project is led by a multidisciplinary team of public health researchers, project specialists, and data analysts and leverages local partnerships with community members and Federally Qualified Health Center, faith-based organizations, community centers, Community Based Organizations, and academic institutions. 

A local Community Advisory Council of key stakeholders in the fields of health care, research, and social services is formed at the start of the project and continues to guide the project and dissemination of findings. 

The work begins with a review of the secondary data sets on health and SDOH of the local communities as well as a review of previously published health assessments or reports.

Focus Groups

Community members are recruited from local health clinics, faith-based organizations, and community-based organizations to participate in focus groups to evaluate components of the community care model such as:
    • Social Determinants of Health (SDOH) assessment
    • Plan care system
    • Technology
    • Delivery method

The findings are co-analyzed, and recommendations are co-developed with community stakeholders and help our partners better understand the impact of community factors on health and wellbeing, including gaps in services and barriers and facilitators to care in the different counties. Through strengths-based and solutions-focused approaches, this work can help guide new programs and initiatives to promote community-driven solutions to health equity for these communities and support a more tailored approach to the local Path Assist delivery.
