How long does it take to implement CareLink?

Implementation timing depends on your organization’s goals, workflows, configuration requirements, data migration, integrations, reporting needs, and number of participating partners. During discovery, we develop a clear implementation plan and go-live schedule. Our team guides you through requirements gathering, platform configuration, integration and data preparation, testing, training, and launch.

Does CareLink integrate with electronic health records and other systems?

Yes. CareLink supports bidirectional integration with EHRs, HMIS platforms, and other health and social-care systems. The right integration approach depends on the systems involved, the information being exchanged, and how that information should support your workflows. Our team works with your technical stakeholders to define data sources, exchange requirements, update frequency, testing, and ongoing monitoring.

Are there special hardware requirements?

CareLink is a web-based, mobile-friendly platform and generally does not require specialized hardware. Authorized users can access the platform through a supported web browser on an internet-connected desktop, laptop, tablet, or compatible mobile device. We review technical requirements during implementation to make sure your devices, browsers, security settings, and network environment are ready for launch.

Will our team receive training on how to use CareLink?

Yes. Training is incorporated into implementation and tailored to the roles and workflows of your organization. Administrators, program leaders, supervisors, and frontline users receive the guidance they need to perform their work in CareLink. We also support testing and go-live preparation so your team can begin using the platform with confidence.

How configurable is CareLink?

CareLink can be configured around your populations, programs, partners, and reporting requirements. This can include standard or custom assessments, care plans, forms, fields, tasks, goals, alerts, automated workflows, access rules, and reports. Our team works with you to translate your operating model into a platform experience that supports the way your organization delivers and measures care.

Does CareLink support medical and non-medical billing?

Yes. CareLink can connect care and service documentation with medical and non-medical billing workflows. Depending on the program and implementation, this can include claims preparation, electronic submission support, payment posting, reconciliation, denial follow-up, and revenue reporting. The goal is to reduce duplicate entry and make billing a more natural extension of the work teams already document.

What support is available after we go live?

Activate Care continues to support customers after implementation. Our team can help troubleshoot issues, answer platform questions, refine configurations, support adoption, and identify opportunities to improve workflows and reporting as programs evolve. The specific support model is defined as part of your CareLink implementation and customer success plan.
