How is this different from a traditional community health needs assessment?

Our assessments can focus on a specific population, geography, program, or benefit—not only a broad community. We combine community-engaged research with operational analysis so findings lead to practical decisions and implementation.

How do you include people with lived experience?

We tailor engagement to the community and can use advisory councils, interviews, focus groups, and surveys. We work with local partners to make sure we have diverse and broad representation across research activities, that participation is accessible, and that participants are engaged in every aspect of the project from implementation to analysis and shaping recommendations.

What research methods does Activate Care use?

Our assessments combine qualitative and quantitative research. Depending on the project, methods may include focus groups, surveys, key stakeholder interviews, Community Advisory Councils, secondary-data analysis, resource mapping, geospatial analysis, and reviews of existing community health assessments and improvement plans.

Can you build on work we have already completed?

Yes. We review existing CHNAs, CHIPs, reports, strategies, and available datasets first. The assessment is designed to close unanswered questions and add decision value—not duplicate good work.

What data can be included?

Depending on the project, we can analyze public health, demographic, clinical, utilization, SDOH, provider capacity, service availability, community resource, survey, interview, and focus-group data.

What will we receive at the end?

Deliverables are tailored to the decision you need to make and may include an executive briefing, findings report, community and partner profile, resource or geospatial maps, priority framework, recommendations, implementation roadmap, and measurement plan.
